I moved from Patreon to your website, but now I lost Discord/FootCast access.

This is an easy fix. You just need to head to FootClan HQ and click the "Connect to Discord" button, (if it shows a current connection, click disconnect and then the connect button after that). This will change the sync to our website and grant you access under your new tier.
For the FootCast, click the "Add to Podcast Player" button, follow the instructions and you'll get the FootCast and/or Injury Blitz (depending on your tier) again.
